FMEA Template (Failure Mode and Effects Analysis)Define the process boundary and use an agreed rating rubric.Process control plan
Define each process check and the reaction when a characteristic is out of control.

What it's for
Define each process check and the reaction when a characteristic is out of control.
What you can download
Available files: PDF, Excel. Each PDF includes the blank form first, a fictional completed example and instructions. The Excel workbook includes an unused template and a separate completed example, with labelled inputs, calculation cells and print settings. The download is free; enter your email to receive a secure link valid for seven days. Product-news consent is optional.
Completed manufacturing example
For operation 20, the example links a 10.00–10.05 mm hole to plug gauge G-12, a first-piece check and checks every 30 parts. An abnormal reading triggers a stop and containment back to the last verified good check, with a named owner.
A control plan specifies the ongoing check and reaction. The FMEA records the risk reasoning; an inspection record retains the actual results. Keep their part, drawing and revision references aligned.
When to use this template
Original manufacturing worksheet. It is not an official AIAG form or evidence of certification.

How to use it
- Link the drawing and process-risk references to each critical characteristic.
- Specify units, sample size and frequency, equipment and the record to retain. Do not leave the reaction plan as “inform supervisor”.
- Review the plan after a change, defect or capability concern. Confirm the measurement method is suitable.
Key features
- Operation and characteristic controls
- Reaction and revision notes
Ideal for
- Process and quality engineers maintaining manufacturing controls
Common uses
- Defining checks and reaction plans before releasing a process
