Risk Assessment Form

The Risk Assessment Form provides a structured approach to identifying workplace hazards, evaluating risk levels, and implementing appropriate controls to protect workers. Also known as Job Hazard Analysis (JHA) or Job Safety Analysis (JSA), this template breaks jobs down into steps, identifies hazards in each step, and applies the hierarchy of controls (elimination, substitution, engineering controls, administrative controls, PPE) to reduce risks to acceptable levels. Risk assessments are fundamental to proactive safety management, required by ISO 45001 and recommended by OSHA, enabling organizations to prevent incidents before they occur through systematic hazard identification and control.
