The Risk Assessment Form provides a structured approach to identifying workplace hazards, evaluating risk levels, and implementing appropriate controls to protect workers. Also known as Job Hazard Analysis (JHA) or Job Safety Analysis (JSA), this template breaks jobs down into steps, identifies hazards in each step, and applies the hierarchy of controls (elimination, substitution, engineering controls, administrative controls, PPE) to reduce risks to acceptable levels.
